Health Department staff will see people of all ages for most chronic diseases. In addition, the diabetes self-management education program has been awarded recognition from the American Diabetes Association. The services include:
One-to-one counseling (chronic diseases including diabetes)
Diabetes group education
Variety of educational classes
Program participation requires a physician referral. Once the referral is received from the physician, the client will be contacted to schedule an appointment.
