Well child physical exams can be performed for children through the age of 20 including: daycare, pre-k, kindergarten, and school physicals. Physicals are performed by a Child Health Nurse Screener.

POSTPARTUM/ NEWBORN HOME VISITS
This program provides home visits within 2 months after hospital delivery to ease the transition to home for both mother and baby. Health history of mom and baby, newborn physical assessment, education, and referrals as needed.

*Postpartum depression screen for mother
*Assessment of newborn including weight
*Lactation information and support
*Nutrition support for mother and baby
*Referrals to other services within the community
Who is Eligible?
Parents who participate in Baby Love Insurance Program or any other interested parent
Appointment Procedure
The Community Transition Coordinator will contact the mother at Onslow Memorial Hospital after delivery.
