Referral Procedures
- Medical entity contacts Home Health and Hospice with a referral.
- Upon receipt of referral, a Registered Nurse will schedule a visit.
Hours of Service
Monday - Friday 8:00 am to 5:00 pm with 24 hour on-call nursing service seven days a week.
